Please note:
- We automatically provide all active members with a Personal Illustration once each year. This document provides details of the value of your accrued benefits at 31 March, the value of your death benefits at 31 March and your prospective benefits at age 65. Your Personal Illustration is normally sufficient to meet your requirements. However, if you need further information, fill in this online form.

- To supplement the Personal Illustration, we will provide one illustration in any 12-month period at the member’s request. We will not accept any further illustration requests from you within the next 12 months. We will not normally provide further illustrations except where benefits are about to become payable or if it is a request for an illustration at age 65.
- If your personal statement request is for a mortgage application or to provide details to a financial advisor, your Personal Illustration should be adequate for this purpose.
- We are happy to provide you with an illustration, but sometimes your employer is often in a better position to provide you with a personal statement as they hold up- to-date hours, pay and membership information for you. Your employer will also be able to tell you what their early retirement and flexible retirement policies are.
- The personal statement we will provide to you will usually be based on your pay for the year ending 31 March, unless it is a request for full disclosure.
- Membership is important in calculating benefits, which is why we have asked you for an assumed leaving date and for details of the present hours you work. If you do not specify an assumed leaving date, we will calculate your prospective benefits using the date you requested a personal statement as your assumed leaving date.
- Voluntary age 50 to 60 retirement request. We can only pay your benefits before age 60 if your employer consents to it. Your employer will have an early retirement policy. Before filling in this online form it is worth checking with your employer what that policy is as it could be that the policy is to not allow voluntary retirement before age 60.
- Ill health retirement. If the date of your retirement due to ill health is on or after 1 April 2008, there are different categories of ill health retirement that you could fall into. The category you fall into will determine whether benefits are payable and, if so, whether they will be enhanced and the level of enhancement. As we will not know which category you may fall into we cannot provide you with an illustration of ill health benefits. If ill health retirement becomes an issue for you, your employer's pensions officer can provide an illustration of your ill health benefits that are specific to your circumstances, or they can request one from us.
- Redundancy or on interest of efficiency retirement request. Your employer will be aware of any early retirement exercises that may be planned. Before filling in this online form it is worth checking with your employer whether any such exercise are in the pipeline and if you would qualify for a redundancy or interests of efficiency retirement. We do not include any extra pension details in the personal statement we provide. The award of extra pension is entirely at your employer’s discretion. If you want us to provide you with a basic personal statement, fill in this online form, selecting either benefit type Redundancy or Interest of efficiency retirement.
- Flexible retirement request. Your employer must consent to the payment of benefits, even if you have reached your normal retirement date. You must be aged 50 or more and you must reduce your working hours or grade (or both). Your employer will have a policy about consent to payment of benefits under the flexible retirement rules. Before filling in this online form it is worth checking with your employer what that policy is and whether you would qualify.
- Full disclosure request. For requests asking for a full disclosure illustration, membership and pay must be at a date within one month of the date the information is supplied. Therefore, we will be required to contact your employer for your current pay details and to confirm your current hours.
